Next to outputting a CSV file, create a human-readable "narrative" of what happened.

For example:
A new patient [PATID] was added to the system at [TIMESTAMP1], [GENDER] born on [DOB].
Patient [PATID] was admitted to [LOCATION1] at [TIMESTAMP2].
A generic patient update message was sent at [TIMESTAMP3] for Patient [PATID].
The patient [PATID]was transferred to [LOCATION2] at [TIMESTAMP4] (coming from [LOCATION1])
A generic patient update message was sent at [TIMESTAMP5] for Patient [PATID].
Patient [PATID] was discharged from [LOCATION2] at [TIMESTAMP2], going to [DISCHARGE DESTINATION].

For the generic patient update messages a08, try to identify if any "relevant" fields from the PID segment were updated (name, address, phone number, etc.)?
